Stepping Home is a new and exciting partnership pilot project, based in West Suffolk, which recognises the impact housing has on the health of its occupants. From adaptations and de-cluttering to heating repairs and access issues, all of these things and many more directly lead to hospital admissions and discharge delays.
East Suffolk Council manage the same project in the Ipswich and East Suffolk Clinical Commissioning Group area, and are excited to be able to extend the scheme to the west of the county with funding from West Suffolk Clinical Commissioning Group.
We are looking for a Housing Discharge Co-ordinator to assist patients who are struggling with a range of housing problems, to find solutions. Significantly altering their healthcare pathway, leading to better health outcomes, while reducing pressure on the local healthcare system.

This varied role requires someone who recognises the importance of partnership working, is able to engage with a range of stakeholders, a proven problem solver, and above all a person who is passionate about helping people.
We are ideally looking for someone with experience in patient discharge, the hospital admission process, or the patient journey, which will enable then to identify and provide intervention to patients whose admission or discharge delay can be avoided by temporarily rehousing them in our brand new unit.
You will have excellent communication skills, be a good team player, have a compassionate attitude and good problem solving abilities.
