Come and join the new 'Connect for Health' service and help benefit communities in the areas of Eye and North West, Stowmarket and South Rural (Shotley area).

It is an exciting time for social prescribing in Suffolk with the development of the new Connect for Health Scheme. Suffolk Family Carers is building on our successful Community Connector service in Shotley and looking for a new team to develop and deliver this service.

The Connect for Health Scheme will support people to improve their health and wellbeing, connect with their local community and find their own solutions.

Do you enjoy developing communities to help reduce isolation and loneliness?

As a Community Connector, you will work 1:1 with people in various settings such as GP practices, on board Suffolk Family Carers' mobile information centre or at their home. We are looking for four Community Connectors with the experience, skills and confidence to engage people and build positive links within local communities.
