Complex Case Team, Mental Health and Learning Disabilities, Adult and Community Services.
This is your chance to take up a unique position in the Suffolk County Council and Norfolk and Suffolk NHS Foundation Trust (NSFT) joint partnership providing mental health services under a S75 Partnership Agreement.
In this key role, you will be responsible for working with adults who are in out of county placements and ensuring regular 3 monthly reviews are carried out and working towards repatriation to Suffolk.
You will be working closely with the community Integrated Delivery Teams. Alongside other professionals you will be expected to comprehensively assess mental health needs and to be able to identify and commission appropriate, resilient and creative care and support packages as well as specialist placements.

It is essential that you have a relevant Social Work qualification and HCPC registration.
You will ideally have a suitable level of demonstrable experience of working in Mental Health Services.
You will be required to travel frequently in rural areas, so you must hold a full valid driving licence and have access to personal transport.
