You will work as part of the professional Social Work service, in this Integrated Neighbourhood Team (INT).
Acting as a role model and leading by example to both unqualified and qualified team members and external colleagues; by playing a key role in promoting personalisation, independence, wellbeing and choice to people. This will include carrying out the most complex asset-based assessments / reviews, short term enablement plans (STEPs), care and support plans and safeguarding plans.
To be sucessful in this role, you will need a strong mix of knowledge and skills and a level of resilience.
Adult Social Work is challenging, but will be continue to be facinating, never dull and very rewarding.
This team provides a range of timely information, support and advice around reablement and personalisation to its customers, to enable them to remain as independent and safe as possible; while working with customers and their carers whose needs are very complex, to assist them with their plan of support.
As a great team member, you will work as part of a team, who will work closely with our colleagues from Home First, Independent Providers, Health, the Boroughs and Districts, the Multi Agency Safeguarding Hub, and a wide range of agencies such as those in the voluntary sector.
In ACS, we work to three areas covering the county of Suffolk, which is called North, Ipswich and East and West localities, with offices based in the towns.
Our community Social Work teams in each locality, are made up of Social Workers, Occupational Therapists and Independence and Wellbeing Practitioners, along with Business Support colleagues.
Each team (aligned to its local GP surgeries) has its own Manager who in turn is supported by an experienced and established senior management team. There is an Area Manager covering each part of the county and additional support is provided by central Service/ Transformation Leads.
Senior Practitioners are employed within each of the four Areas and they cover operational practice and professional development, linking into the county Professional Advisors. An Allocation Lead ensures that workflow within each Area is effective and a Finance & Performance Manager completes the set-up, working with the teams to ensure that the budgets are on track. We are led by our Assistant Director who reports to the Director for ACS.
A relevant social work qualification and HCPC registration is essential, along with continued professional development.
You will be required to travel frequently in rural areas, so you must hold a full driving licence and have access to personal transport.
A conditional relocation package of up to £8k is available to you, to support a potential move to Suffolk.

The salary range is in line with our ACS (Adult Community Services) Social Work Services Progression Scheme.
